Register with us as a New Patient

Would you like to register with us at our General Practice: Moxley Medical Centre?

We are a small practice providing healthcare as part of the National Health Service. If you want to register as a patient with us, please download the following forms found further down the page. If you have any difficulty in printing or filling in the form, please get in touch with reception at 01902 409515. Thank you!

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

Practice Area

We are pleased to accept registration requests from patients who are living within the area indicated in the map below. Please enter your postcode within the “Postcode check” box to find out if you live within our catchment area. If you are unsure whether you are in our catchment area, please contact reception at 01902 409515.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes.  All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

We have the right to refuse to register patients if they have a violent flag against their record.

Registration Process

In order to register at our Practice, please fill in the following documents:

  1. GMS1 Form
  2. Summary Care Record (SCR Consent Form)
  3. Health check form for Adult/Child
  4. Two forms of ID (Please see Registration Policy for further details)

These forms can be downloaded through the link below or can be collected at Reception if you require assistance. Please ensure you complete the form with the correct details following our Registration Policy and return it to Reception after completion. We will answer whether your application has been accepted or not within 7-10 days.

When a patient registers with a Practice, it can take some time for medical notes to be transferred from the previous practice. The form must be completed accurately so we can monitor any conditions appropriately.

We also request to book an appointment for a new patient check-up to complete the registration process.
